Injectable Implant Consent & Waiver of Liability Overview

Please read the following policies and risks carefully. You will be required to fill out and sign a physical copy of this waiver in person prior to your procedure. You must be 18+ with a valid government-issued photo ID.
Medical History Requirements
You will be asked to disclose any of the following conditions prior to the procedure, as they may require physician approval:
- Heart conditions or use of a pacemaker
- Diabetes
- Hemophilia, bleeding disorders, or use of blood-thinning medication
- Hepatitis, HIV/AIDS, or other communicable diseases
- Autoimmune disorders
- Allergies to metals, silicone, bioglass, latex, or skin antiseptics
- Eczema, psoriasis, or active skin conditions at the procedure site
- Proneness to keloid scarring
- Currently pregnant or nursing
Informed Consent & Assumption of Risk
Not a Medical Procedure: This procedure is a form of body modification performed by a professional artist, not a licensed medical professional. It is not being performed for medical purposes.
Permanence, Removal & Scarring: This is a permanent modification. While Megan Paige offers removal of the implant free of charge if it is ever needed, the extraction process requires a physical incision (cut) into the skin to remove the object, which will result in permanent scarring.
Infection & Healing: Infection is always possible with any invasive procedure. You must agree to follow all provided aftercare instructions. Any medical complications requiring outside treatment due to your own negligence will be at your own expense and risk.
Rejection & Migration: The human body may naturally reject a foreign object, pushing it out of the skin, or the implant may migrate from its original placement, which can cause discomfort or require removal.
Under the Influence: You cannot be under the influence of alcohol, drugs, or any substances that could impair your judgment or thin your blood at the time of the procedure.
Specific Risks for Microchips & Magnetic Implants
Medical Imaging / MRIs: Magnetic implants and certain microchips may interfere with Magnetic Resonance Imaging (MRI) and other medical scans. You accept full responsibility for informing allmedical professionals about your implant prior to any imaging or procedure.
Bioglass Breakage (Microchips): Microchips are often encased in bioglass. Severe impact, pressure, or trauma to the implant site can cause the glass to break under the skin, requiring immediate medical removal.
Pinching & Crush Hazards (Magnets): Magnetic implants are strongly attracted to ferrous metals. This attraction can cause the implant to pinch the skin, catch on objects, or crush tissue if caught between a strong magnet and a heavy metal object.
Data & Technology Lifespan: Megan Paige makes no guarantees regarding the lifespan, read-range, or technological compatibility of the microchip with third-party software, hardware, or readers. However, manufacturers of these devices (such as Dangerous Things) often offer a warranty on the hardware itself. Any warranty claims or hardware failures must be handled directly with the manufacturer.
